Chronic Disease Management – Yarra Glen

Living with a long-term health condition can be challenging, but you don't have to manage it alone. Our GPs and nurses work with you to create a plan that helps you stay as healthy and independent as possible. Chronic disease management is bulk billed for Medicare card holders.

Conditions we help with:

A chronic condition is a health condition that has lasted, or is likely to last, six months or more. Our GPs help people living with:

​

- Diabetes

- Heart disease and high blood pressure

- Asthma and COPD (chronic lung disease)

- Arthritis and chronic pain

- Kidney disease

- Osteoporosis

- Mental health conditions

- Other long-term conditions

Care plans:

GP Chronic Condition Management Plans

 

If you have a chronic condition, your GP can prepare a GP Chronic Condition Management Plan with you. Your plan sets out:

- Your health goals

- The treatment and care you need

- The health professionals who will support you

- The steps you can take to manage your health

 

Our practice nurse may help your GP prepare your plan and support you along the way. Your GP will review your plan with you regularly to see how you're going and make changes if needed.

Allied health:

Access to allied health services

 

With a GP Chronic Condition Management Plan, your GP can refer you for Medicare-rebated allied health services, such as physiotherapy, podiatry, dietetics, exercise physiology or diabetes education. You can usually access up to 5 rebated allied health sessions each calendar year.

 

Your plan needs to be current for you to keep accessing these rebates, so it's important to keep your review appointments.

To receive Medicare rebates for a GP Chronic Condition Management Plan, you need to see your usual GP or be registered with our practice through MyMedicare.

 

Registering is free and helps us provide the best ongoing care. Ask reception how to register, or visit our MyMedicare page.
